*Acknowledgment of Risk

 

I understand that participation in The BiteSize or Next Size Factory Youth Basketball League involves physical activity and that injuries may occur. By signing below, I acknowledge these risks and voluntarily allow my child to participate. ( “I Agree”)

 

*Medical Authorization

 

In case of emergency, I authorize The BiteSize or NextSize Factory staff to seek medical attention for my child. (“I Agree”)

 

*Release of Liability

 

I hereby release and discharge The BiteSize and NextSize  Factory, its staff, and facilities from any claims related to injury, illness, or property damage arising from participation. (“I Agree”)

 

*Code of Conduct Agreement

 

I understand that participants and parents must follow all league rules and codes of conduct. Violations may result in removal from the league. (“I Agree”)

 

*Photo & Video Release

 

I grant permission for my child’s image to be used in promotional materials. ("I Agree”)

 

*COVID-19 & Health Acknowledgment

 

I acknowledge the risks associated with participating in group sports and agree to follow all health guidelines. (C“I Agree”)
